This information is needed in order for us to identify and register you (your family and friends) for the COVID-19 vaccine and to communicate with you.
This personal information will be held on a NSW Health storage cloud.
Before and during the visit, the information will be accessible by NSW Health staff such as the clinic nurses and clerical staff members and Support teams to manage the visit, confirm identities and to contact you/your Person to Contact.
This information will be transferred to your personal health record held at NSW Health.
By using our Online Services, you agree that you will not engage in any conduct that is: disruptive or detrimental to NSW Health; or disruptive or detrimental to the operation and functionality of our Online Services; or unlawful. NSW Health will use its best efforts to ensure stable and secure access to the registration system, but does not guarantee that the access will always be free of errors, virus, interception or interference.
If NSW Health becomes aware that use is in contravention of these terms and conditions, it reserves the right to immediately terminate and block the access without prior notice.
3. Liability and Indemnity
By using NSW Health COVID-19 Vaccination Registration Forms you release and discharge NSW Health from any liability which might arise from the use of the service including liability in relation to defamatory or offensive material or any breach of the copyright which may occur as a result of use.
