NSW privacy laws will apply to any information (whether personal or personal health information) which NSW Health collects from individuals when they use our website. We are committed to treating your personal information in accordance with privacy law.

Whilst using the NSW Health COVID-19 Vaccination Registration Form, we are required to collect personal information about you (your family and friends) including, but not limited to, your full name, date of birth, your contact details, some medical information and details about your current visit. This information is needed in order for us to identify and register you (your family and friends) for the COVID-19 vaccine and to communicate with you.

This personal information will be held on a NSW Health storage cloud.

Before and during the visit, the information will be accessible by NSW Health staff such as the clinic nurses and clerical staff members and Support teams to manage the visit, confirm identities and to contact you/your Person to Contact.

This information will be transferred to your personal health record held at NSW Health.

1. Terms of Use

By using our Online Services, you agree that you will not engage in any conduct that is: disruptive or detrimental to NSW Health; or disruptive or detrimental to the operation and functionality of our Online Services; or unlawful. You will not Infringe NSW Health's intellectual property rights use the service for commercial activities, gambling, use the service to upload/download large files use the service to harass or discriminate against anyone or invade their privacy use the service to access or distribute offensive material, or material you do not have rights to use the service to become involved in destructive activities such as sending viruses or spam, use data gathering, monitoring, extraction or hacking tools, including (but not limited to) data mining, spiders or robots alter your messages to make it look like they don't come from you interfere with our network, or access any other network equipment or data without authorisation use our services for any purpose that has an unreasonable load on the infrastructure, or that has the effect of undermining the integrity of our services or damaging or disrupting our services.

2. Conditions of Use

NSW Health will use its best efforts to ensure stable and secure access to the registration system, but does not guarantee that the access will always be free of errors, virus, interception or interference.

If NSW Health becomes aware that use is in contravention of these terms and conditions, it reserves the right to immediately terminate and block the access without prior notice.

3. Liability and Indemnity

By using NSW Health COVID-19 Vaccination Registration Forms you release and discharge NSW Health from any liability which might arise from the use of the service including liability in relation to defamatory or offensive material or any breach of the copyright which may occur as a result of use.

You agree to indemnify and must defend and hold harmless the NSW Health, its employees and agents, from and against all loss, damage, liability, charge, expense (including all reasonable legal and other professional costs on a full indemnity basis) of any nature or kind arising from your breach of these terms and conditions.

4. Termination and Variation

We can terminate your use of and/or access to our Online Services at any time at our own discretion. We may vary these Terms of Use at any time. If we do so, we shall post new Terms of Use on our website and will draw any changes to your attention if it is practical and reasonable to do so.
